Social Connections Program

Social Connections is a peer led program to assist community members who are feeling isolated and lonely due to COVID-19.

Our Social Connections Program pairs you with a Peer Connector who provides you with the support to help you engage into community based interests, goals and social needs.

You don’t need to have a previous diagnosis of mental illness, or treatment – this program is for anyone. We are here to assist you to link with your area of interest.

  • Co-create and co-deliver an action plan
