BCH Medical History Requests
Ballarat Community Health will provide a Medical History Summary to all clinic patients upon request, free of charge.
A summary is different to a Complete Medical Record, which includes all clinical notes, correspondence, referrals and results, and is usually in excess of 250 pages of information. This information is collated from multiple sources and reviewed to ensure that the correct information is provided. This can take several hours to complete for each record, and it may take up to one month for a complete medical record to be reviewed.
Due to the time required for preparation and review, requests for a Complete Medical Record will incur the following fees:
- $50 for non-concession card holders
- $20 for concession card holders and individuals below the age of 16 years

You can only request your own medical records and must provide proof of your identity.
If you are requesting medical records for another person, you must provide proof that you are authorised to request this information on their behalf (eg: medical treatment decision maker, parent or guardian).
