What is Mental Health Service Navigation?

The Mental Health Service Navigation connects you to an experienced, local clinician who can assist you through the following:

• Learning and tailoring access to the most appropriate service to meet your needs
• Focus on improving your mental health, physical health and social needs
• Help you to connect socially/with the community
• provide information and support to carers and family members about local services and supports.

The clinician works closely with local service providers which establishes coordinated referral pathways to allow for ease of access for consumers.

Who can access?

• Available to those in Western Victorian PHN region
• Consumers experiencing mental health conditions
• Family members or carers
• Professionals working in human service organisations, GP’s and allied health professionals
