Trans & gender diverse services

The Victorian Government has funded an initiative to increase health services across Victoria for trans, gender diverse and non-binary people aged 18 and over.

Ballarat Community Health is working in partnership with Your Community Health, Austin Health and Thorne Harbour Health.

We aim to provide gender affirmative healthcare services and it all starts with making an appointment with one of our peer navigators. A peer navigator is a person with lived experience who also has the professional skills to support you to create pathways to affirm your gender and access health services depending on your personal needs, goals and where you are located. Our peer navigators are able to support all trans, gender diverse and non-binary Victorians with information, support and referral to health services.

Multi-disciplinary services are available in Ballarat and linked to our Lucas, Wendouree and Sebastopol sites and delivered in partnership with Your Community Health in Preston, alongside specialist support from Austin Health.

A number of our services can also be accessed via telehealth or phone, so if you can’t get to our sites in person we are still able to help you by either providing services online, by phone or by referring you to a health service in your local area.

Whatever your goals are, whether it be to socially, medically or legally affirm your gender, or simply just to find a healthcare provider who can provide general health services, please fill out the contact form below to receive a call back or more information.

To access services and be connected to a Peer Navigator, please complete the Trans and Gender Diverse Initiative service access form and we will get back to you as soon as possible.
