Integrated Chronic Disease Management Counsellor

Chronic diseases can impact every facet of a person’s life, causing a significant amount of stress upon the individual, families/carers. At Ballarat Community Health our Integrated Chronic Disease management Counsellor aims to assist individuals with chronic diseases. This service focuses on mental health and care management from a wellness and reablement perspective. This service can include counselling, support, health coaching and advocacy for individuals and families living with a chronic health condition (diabetes, musculoskeletal conditions, multiple sclerosis, Parkinson’s disease, Heart Failure etc.).

The service can include assistance with

  • Mental Health Counselling
  • Psycho-Social Support
  • Behaviour change and goal setting
  • Stress management and enhancing wellbeing
  • Family and relationships issues
  • Grief and loss issues
  • Pain management
  • Learning about self-management principles
  • Psychosocial assessment
  • Illness and role adjustment
  • Secondary symptoms of chronic illness (e.g. low mood, worry, anger or sleep disturbance)
  • Socio-legal concerns and medical decision making

Life! Program

Life! Program

Reduce your risk of type 2 diabetes, heart disease and stroke through our free lifestyle modification program.

BCH runs Life! groups year-round at no cost to eligible participants.

Who Can Join

You’re eligible if you’re 18+ and meet ONE of these criteria.

Health Risk Factors

  • AUSDRISK score 12 AND BMI m2 (Asian background BMI 23kg/m2)
  • Previous diagnosis of cardiovascular disease, gestational diabetes, chronic kidney disease, familial hypercholesterolemia, high cholesterol (>7.5 mmol/L), high blood pressure (Systolic 180mmHg or Diastolic 110mmHg), pre-diabetes, or PCOS

Age-Based Risk

45+ years (or 35+ for Aboriginal/Torres Strait Islander people) with Absolute Risk score ?10% (GP referral required)

What’s Included

  • 1 one-hour individual session
  • 5 group sessions (90 minutes each) over 6 months
  • 1 follow-up session at 12 months
  • Comprehensive workbook covering nutrition, exercise, goal setting, sleep, and stress management
  • Wednesday morning and Friday afternoon sessions available
  • Partner/friend welcome for support
  • All sessions led by qualified health professionals

Who Cannot Join?

  • People with existing diabetes
  • Pregnant women
  • Recent (last 3 months): angina, angioplasty, active cancer, or heart attack

Required Tests

Recent blood tests (within 12 months) must show
  • Fasting blood glucose (plus glucose tolerance test if FBG ?5.5mmol/L) or HbA1c
  • Cholesterol panel (total, HDL, LDL, triglycerides)
Copies required for program facilitators.

Who are Diabetes Educators?

Diabetes educators are specially trained health professionals who help you understand and manage your condition. You can self-refer or get a referral from your doctor.

Available at Sebastopol, Wendouree (Howitt Street), and Lucas sites
Appointment length 30-90 minutes
How often Depends on your needs – from regular visits to yearly check-ups

How we help

Our diabetes educators support you with

  • Understanding your diagnosis
  • Starting or adjusting insulin and medications
  • Managing blood sugar highs and lows
  • Learning about healthy eating
  • Handling diabetes at work, school, or while traveling
  • Connecting with other health professionals
  • Supporting family and caregivers

What to Expect

During appointments, we’ll discuss the topics that matter most to you – from daily management and medication to lifestyle changes and preventing complications.

Additional Support

We also have dietitians, exercise physiologists, podiatrists, counsellors, and onsite gyms to support your health journey. You can self-refer to any of these services.
